ANALYTICAL TESTING REQUEST FORMS

Analysis Request Forms or chain of custody forms are used to identify samples being sent, date and times of collection, the analyses requested and to whom results should be sent.

Please use the following Analysis Request Forms (ARFs) when submitting samples for analyses:

1. Food safety samples

2. Pharmaceutical samples

3. Agrochemical/Residue Samples

Laboratory Hours of Operation

The lab’s normal hours of operation are Monday through Friday from 8 AM to 5 PM (EST). Please do not send samples for weekend delivery unless prior authorization has been requested.

Holding Times

A sample’s holding time is the maximum length of time from sample collection to sample analysis. Different methods have different holding times. Please check with the lab if you have any questions regarding this at (904) 645-9169.

Payment Terms

A credit card or payment in advance is required for new customers. To arrange payment or if you have any questions, please contact the lab at (904) 645-9169.

ISO/IEC 17025 ACCREDITED

ISO/IEC 17025 is a company level accreditation based on a standard published by the International Organization for Standardization (ISO) titled “General requirements for the competence of testing and calibration laboratories”. ISO/IEC 17025 is a general umbrella term used to refer to the specific standard; the current version of this standard is ISO/IEC 17025:2017. The standard contains two main sections: management requirements (primarily related to the operation and effectiveness of the quality management system within the laboratory) and technical requirements (primarily related to the competence of staff and calibration of equipment). The standard also gives requirements related to quality management such as document control and corrective action. This standard is the basis for accreditation from an accrediting body.

ADPEN has also been inspected by the US-EPA and US-FDA multiple times with no observations.
